Your medical records provide the history of your treatment at Coshocton Hospital including diagnosis, surgery or treatments performed, medications given and any other records that pertain to your medical care. Our physicians, nurses and other healthcare clinicians provide the detail of care that comprises your record, which stands as an important resource for information when future health care services are needed. Our medical record staff serves as a communication link among these caregivers, so your record of treatment is effectively maintained. These medical records professionals also ensure that all of your personal health information is kept private in accordance with federal and state laws.
How to Obtain Copies of Your Medical Records
As a patient, you have the right to access your medical records. To obtain copies, we require that you complete a signed release-of-information form, as well as photo identification. Please call us at (740) 623-4121 for instructions on obtaining and completing the form. Since privacy regulations designate that only you (the patient) or your legal representative may obtain access to medical records, we require verification of legal representation before the records can be released to your representative.
Fees for Copying Medical Records
You may be charged a small fee for obtaining copies of your records, depending upon the reason you wish to receive copies. If there is a charge, we will inform you before any copies are made. General copy fees are as follows: For pages 1 to 10, $2.88 is charged per page; for pages 11 to 50, the charge is $.60 per page; and for pages 51 and over, the charge is $. 24 per page.
